RECOMBINANT IMMUNOBLOT - RIBA II GENERATION TEST IN HCV INFECTION DIAGNOSIS

Abstract

Immunoblot RIBA test has a function of confirmatory assay in HCV infection diagnosis. RIBA II generation test detects antibodies against recombinant HCV antigens - C22-3, C33-c, C100-3 and 5-1-1. Aim of the work was to determine serologic status in patients with acute and chronic HCV infection with RIBA II generation test, and to establish the significance of this test in differential diagnosis of acute and chronic HCV infection. Investigation included 37 patients with acute and 23 patients with chronic HCV infection.
In 94,60% of those with acute HCV infection, RIBA test was positive 2 months after the occurence of clinical symptoms of hepatitis and higher values of transaminases, while 5,4% of the diseased were RIBA intermittent. All RIBA reactive subjects had antibodies against the structural antigen C22-3 and non-structural antigen C33-c. In 80%, detected were anti C1OO-3 too, and in 57,14% anti 5-1-1 antibodies. In 5,40% detected were only anti C22-3 antibodies, and because of that they were RIBA intermittent. Supposing that the seroconversion of other antibodies will occur later, the follow-up process was continued. One month later, testing demonstrates anti C33-c antibodies, i.e. RIBA test was positive. All subjects with acute HCV infection had antibodies against structural antigen C22-3, 94,59% anti C33-c, 75,67% anti C100-3, and 54,05% anti 5-1-1 antibodies.
Based on the obtainded results, determined were 4 types of marker status in patients þvith acute HCV infection 2 months after the occurence of clinical symptoms and higher values of transaminases: I. anti C22-3 antibodies, II. anti C22-3 and anti C33-c, III. anti C22-3, anti C33-c and anti C100-3 antibodies, IV. anti C22-3, anti C33-c, anti C100-3 and anti 5-1-1 antibodies. Level of anti C22-3 and anti C33-c antibodies was higher (3+ to 4+) in relation to the level of anti C100-3 and anti 5-1-1 antibodies (1+ to 2+).
In all patients with chronic HCV infection detected were the antibodies against all recombinant HCV antigens. Levels of anti C22-3, anti C33-c, anti C100-3 and anti 5-1-1 antibodies were high (3+ to 4+).
