PELVIMETRY AND BODY MASS INDEX IN BREECH DELIVERIES

Prognosis for both mother and baby is more uncertain in breech presentation compared to the head presentation in the front neck position. Great dilemma and danger in vaginal breech delivery is in the estimation of the existence of possible cephalo-pelvic disproportion. Because of this, an adequate external examination is needed and determination of breech measures. In very obese women, the following must be taken into account: bigger deposits of fatty tissue in the pelvis, and accordingly, decreasing of the pelvic area. The basic aim of the research was to determine whether breech measures and normal BMI influence the breech delivery occurence. The conducted case control study included 564 mothers who delivered babies at the Obstetrics and Gynecology Clinic, Clinical Centre Kragujevac, in the period from January 1, 2000 to December 12, 2002. Of them, 282 had breech deliveries and 282 had head presentation deliveries. Only singleton pregnancies were considered. The following parameters were examined: bispinal diameter, bicrystalline diameter, bitrohanteric diameter, conjugata externa and normal BMI - BMI (Body Mass Index). Significantly the highest possibility of BI SP measures is 21-25 cm, 30% have higher values, and 5% have lower values. Significantly higher possibility of BI CR measures, in both groups, ranges from 27-28 cm (83%). Possibilities are equal for BI TR measures up to 30 cm and from 31-32 cm. 24-27 % of examined women from both groups have values of the measures C Ex less than 18 cm.
