The preset study describes the functional lower lip reconstruction by the mental V-Y island neurovascular advancement flap. In the male patient 84 years of age, bleeding planocellular lateral lip cancer was diagnosed. Advancement V-Y island neurovascular muscle-cutaneous flap, nourished and innervated by mental arteria and nerv was used in lip reconstruction. The remnants of the orbicularis oris muscle, as well as the depressor anguli oris muscle and depressor labii inferioris muscle with their motor branches of the facial nerve were used for the orbicular sphincter reconstruction. The vermilion was reconstructed by the local mucosal flap. The patient was discharged after seven days with completely vital V-Y island neurovascular muscle-cutaneous flap for home care.
